How do we perform when it comes to the out of hospital programme in GP practices
Our CCG is made up of GP practices who are engaged in working together. We regularly take part in a series of engagement events with patients and other stakeholders to explore how we implement the NHS General Practice Forward View (GPFV), which was launched by NHS England in 2016.
Following publication of the GP Five Year Forward View from which we developed our primary medical care commissioning strategy for NHS Bradford City CCG and NHS Bradford Districts CCG, with the following priorities:
To take these themes forward, we are encouraging opportunities for health care professionals to work together differently. Many local GP practices are choosing to work more closely together and improve services and quality for the patients. For example, we are doing practical things like accessing capital funding to improve GP surgeries and increasing opening times in the evenings and at weekends.
Care Navigation training for practice staff: We are training all practice reception staff in Care Navigation by March 2019. Care navigation is a person centred approach which uses signposting and information to help primary care patients and their carers move through the health and social care system as smoothly as possible. It also saves GP time, which can be spent caring for patients with more complex needs. You can find more information on the self-care and prevention page.
